I am not a preferred provider for any health insurance plan and do not accept insurance. I am happy to provide you with a receipt for your insurance company.

If you need to cancel or reschedule an appointment, please call at least 24 hours in advance to allow others the opportunity to schedule. Any appointment canceled less than 24 hours in advance is billed at the full rate.
